Bukidnon II Electric Cooperative, Inc. (BUSECO) • Online Application for Membership

1. Scope of Personal and Sensitive Personal Information Collected

In accordance with the Data Privacy Act of 2012 (Republic Act No. 10173), BUSECO collects and processes ONLY the specific data submitted in the Membership Application Form, categorized as follows:

Personal Information (Section 3(g), RA 10173)

  • Full Name: First Name, Middle Name, Last Name, and Suffix (JR, SR, II, III).
  • Complete Service Address: Sitio, Barangay, and Municipality within BUSECO coverage.
  • Email Address: Used for official PMES seminar link, exam credentials, and payment confirmations.
  • Mobile Phone Number: In the format 09XX-XXX-XXXX for transactional SMS, service connection updates, and account verification.

Sensitive Personal Information (Section 3(l), RA 10173)

  • Valid Government-Issued ID: Scanned copy or photograph of your valid government identification (e.g., PhilID / National ID, Driver's License, UMID, Passport, PRC ID, Voter's ID) containing government ID numbers and verification features (Sec. 3(l)(3)).
  • 2x2 ID Facial Photograph: Recent official 2x2 photograph exhibiting your facial portrait, utilized exclusively for biometric visual verification and official cooperative membership identification.
Strict Data Limitation Notice ("No Others"): BUSECO collects ONLY the personal details and the two sensitive personal information items listed above. BUSECO does NOT collect, request, or process any other unrelated sensitive personal information, such as your race, ethnic origin, religious or political affiliations, marital status, health, genetics, sexual life, or court/criminal proceedings.

2. Purpose for Processing Collected Data

Pursuant to Section 12 and Section 13(a) of the Data Privacy Act of 2012, your personal and sensitive personal information is collected and processed exclusively for the following legitimate purposes:

  • • Identity Authentication & Verification: To cross-verify your full legal name, facial photograph, and government ID to prevent identity theft, impersonation, or duplicate membership records.
  • • Geographic Connection & Franchise Validation: To evaluate your service address (Sitio, Barangay, Municipality) for electric service feasibility and district assignment within BUSECO coverage.
  • • MCO Qualification & PMES Certification: To establish your qualification as a Member-Consumer-Owner (MCO) and issue your official Certificate of Attendance.
  • • Transactional & Operational Communications: To transmit your Payment Reference Number, PMES seminar link, payment confirmation, and electric connection status updates.

3. Confidentiality, Security & Storage Safeguards

All collected personal information, uploaded IDs, and photos are treated with strict confidentiality. Data is stored on secure, encrypted servers with access restricted solely to authorized BUSECO verification and membership personnel.

BUSECO shall never sell, lease, transfer, or disclose your personal or sensitive personal information to third parties, advertising entities, or commercial platforms without your explicit prior written consent, unless required by lawful government authorities or court orders.

4. Your Statutory Rights under RA 10173

As a Data Subject, you are entitled to the following rights under the Data Privacy Act of 2012:

• Right to be Informed of how your data is collected and processed.
• Right to Access your submitted personal files and records upon request.
• Right to Rectification to dispute and correct any inaccurate information.
• Right to Object / Erasure pursuant to lawful grounds under the law.
